Home care Nurse (RN)

Cornerstone Healthcare is seeking a reliable Registered Nurse (RN) to provide skilled nursing visits and supervise CHHAs in the field. The RN will complete assessments, update care plans, and ensure high‑quality home care services.

Requirements:

  • Active NJ RN license
  • 1 year of experience preferred
  • Strong assessment and documentation skills

Home care Nurse (LPN)

We are hiring a dedicated Licensed Practical Nurse (LPN) to provide in‑home nursing care under RN supervision. Responsibilities include vital sign monitoring, medication administration, wound care, and accurate documentation.

Requirements:

  • Active NJ LPN license
  • 1 year of experience preferred
  • Strong communication skills

Certified Home Health Aide Nurse (CHHA)

Cornerstone Healthcare is looking for compassionate Home Health Aides to assist clients with personal care, light housekeeping, meal prep, and companionship. CHHAs follow the care plan and report changes to the RN.
